| In order for us to provide you the best care possible, please bring all of the following with you to your scheduled appointment. To expedite your appointment, all of our NEW PATIENT FORMS are available to you on the left side of this page. You can print these forms off, fill them out at your convenience, and bring them with you on your first visit along with the items listed below. |

| Insurance |

| As a courtesy, we will gladly file your insurance for you. Please remember that you are responsible for all co-payments, co-insurance, and/or deductibles at the time of service. If your insurance provider requires a referral (Medicaid or HMO) it is the patients responsibility to make sure we receive that referral before your scheduled appointment. If we do NOT get your referral, we can see you as SELF PAY, or reschedule your appointment for another day. Our offices accept cash, checks, and credit/debit cards as forms of payment. |

| Our Locations |

| For our patients’ convenience we offer our services at 3 locations in Northeast Georgia. All of our physicians treat patients at each of these locations. Our main office and Ambulatory Surgery Center is located in the heart of Cumming. We have a second location in South Hall County just north of Friendship Road, and our third location is in Duluth, directly across the street from Duluth Medical Center-Duluth. To schedule an appointment at any of these locations just call our appointment line at 866-431-1577. Our office hours are Monday – Friday 8am – 5pm. |
